1. Perform hand hygiene.
2. Identify the patient.
3. Close door or bed curtains and explain the procedure to
the patient.
4. Help patient to chair if possible. Elevate head of bed for
bedridden patient.
5. Fill washbasin with warm water (100° to 104°F [37.7° to
40°C]). Place waterproof pad under basin. Soak patient’s
hands or feet in basin. Do not soak the feet of a patient
with diabetes.
6. Place call light within reach. Allow hands or feet to soak
for 10 to 20 minutes.
7. Dry the hand or foot that has been soaking. Rewarm
water, and allow other extremity to soak while you work
on the softened nails.
8. Gently clean under nails with cuticle stick. If nails are thick-
ened and yellow, patient may have a fungal infection. Wear
disposable gloves and eye protection.
9. Beginning with large toe or thumb, cut nail straight across.
Shape nail with file. File rather than cut nails of patients
with diabetes or circulatory problems.
10. Push back cuticle gently with cuticle stick.
11. Repeat procedure with other nails.
12. Rinse foot or hand in warm water.
13. Dry thoroughly with towel, especially between digits.
14. Apply lotion to hands or feet. Do not apply lotion between
the toes of a patient with diabetes.
15. Help patient to comfortable position.
16. Remove and dispose of equipment.
17. Perform hand hygiene.
